• Receive Determination Call

  Please allow 2-3 weeks for a clinician to review your new patient form. Due to the high volume of new patient requests, the review process may take longer.

• Schedule Appointment

  If we can meet your specific treatment needs, we will contact you using the phone number listed in your new patient form to schedule your first appointment.
